Works with and enables patients on the necessary lifestyle changes involved in a diabetes disease diagnosis, including nutrition, medication, physical activity and glucose testing.
Provide individualized care partnership to patients on how to monitor their blood sugar levels and adjust their diets accordingly
Reviews patient records and meets regularly with patients to ensure they are managing and reaching their agreed goals ( treatment, lifestyle, etc)
Coordinate and facilitate advocacy support groups for patients who have been diagnosed with diabetes or other endocrine disorders
Performs, and enables patient's clinical assessments, such as measuring height and weight, taking blood pressure, checking feet for signs of injury or infection, and administering tests for glycated hemoglobin levels as part of the patient's management of the disease process
Works with patients on how to manage their disease by following a healthy diet plan and getting regular exercise
Works with patients contributing to their education on how to inject insulin or other medications using proper techniques to avoid infections or other complications
Coaching, teaching, mentoring patients on basic principles of nutrition, meal planning, food selection, vitamins and minerals, and how to read food labels
