Duties and Responsibilities:

The Phlebotomist will do, but not limited, to the following:
Identify patients correctly.
Identify out-patients of all ages by requesting the patient (or guardian) to state his/ her full name, file number and comparing with the name & number on the requisition sticker.
Identify in-patients of all ages by comparing name and medical record number on requisition with name and medical record number on the patients wristband before a procedure.
Perform blood draws competently and expediently with as little patient discomfort as possible.
Verify and collect proper samples required for specific tests.
Performs bleeding and clotting time and noting the result.
Knows what tubes are required for various tests, what handling is necessary for each and how to find out when it is not known Label and organize all tests appropriately to make sure that they are easily retrievable.
Label samples with accuracy after extraction.
Ensures that samples are dispatched inside the laboratory in a timely fashion
Maintains collection awareness of timed-sensitive samples (from collection to delivery of samples inside laboratory) such as Ammonia and Lactate.
Maintains the integrity of the specimens by following department procedures, using aseptic technique and observing isolation procedures.
Tracks collected specimens by dating, initialing and noting the time of collection and maintains the daily tallies of the collection performed.
Demonstrates competence in all procedures required of a phlebotomist in accordance with institutional policies; with particular emphasis on blood samples, blood cultures, referral tests and specimen requirements.

Notifies the section supervisors of any significant patient issues.
Notifies section heads of unusual orders for further clarification to ordering physicians.
Practicing infection control standards at all times when working with patients, equipments and/or devices.
Enforces and follows all laboratory safety rules and reports any possible hazards to the head of the department.
Shares appropriate communication with other laboratory personnel involving difficult venipuncture, timed-sensitive samples, etc.
Assists in orientation of new staffs in phlebotomy area as requested.
Shows ability to handle unexpected situations and is able to exercise independent thoughtand actions when necessary.
Accepts responsibility for and becomes available to work a different work shift whenworkload/or other conditions warrant.
Keeping phlebotomy area well-organized at all times.
Keep blood drawing trays neat, clean and well stocked with supplies.
Maintains an adequate supply of specimen collection supplies (sample collection tubes/ containers, syringes/ needles, etc.)
Perform other duties as requested.
Responsible for own professional development and growth and remains updated in current professional practices.
Participates in CQIPS System Improvement Activities, CQIPS System Control Activities, CQIPS Staff Development Activities.
