NURSES/CAREGIVER/ASSISTANT NURSE

Job Description

Description:
-Monitoring the client's health status and adjusting care plans as needed on client needs
-Working with the family to ensure that they are comfortable with the care plan and know how to provide care if needed
-Scheduling appointments with physicians and other healthcare professionals to ensure proper treatment is being received
-Ensuring that the home environment is safe for the client by ordering new equipment or making repairs to items such as stairs or cabinets
-Coordinating with other service providers such as physical therapists, occupational therapists, and social workers
-Assessing the client's needs and working with the family to develop a care plan that meet those needs
-Managing all aspects of the client's care, such as hiring and training caregivers, helping with bathing and dressing, preparing meals, and administering medications
-Cultivating strong relationships with the client and his or her family in order to understand their needs and concerns
-Communicating regularly with case managers and other home care staff members about the client's progress and needs
