PATIENT SAFETY SPECIALIST
Job Description
Summary
Assumes responsibility for the planning, organization, facilitation and implementation of Patient Safety activities. Coordinates the activities of the Patient Safety section, as directed.Essential Responsibilities and Duties
2.Participates in the development of Quality Management (QM) Department policies and procedures, curriculum for education and training in alignment with the patient safety plan.
3.Promotes Hospital-wide awareness of patient safety concepts and encourages participation and involvement of leadership.
4.Supports concerned departments in designing patient safety and risk management programs within their department, as assigned.
5.Collaborates with QM departments and sections in identifying areas for improvement that promotes patient safety and facilitates the efforts to implement the improvement.
6.Collaborates with all relevant services within the Hospital to identify educational needs in relation to patient safety.
7.Educates Hospital staff, including senior management and medical staff on patient safety, as assigned.
8.Promotes Just Culture where errors and near-misses are openly discussed and used as learning opportunities.
9.Oversees activities related to data collection, data review, analysis and dissemination of patient safety information.
10.Prepares and presents quarterly reports on adverse events and identified trended issues and risk areas to Director, QM.
11.Assess the safety culture of the organization and helps in designing action plans for improvement. Participates in performance improvement activities as related to patient safety.
12.Facilitates Hospital compliance with patient safety related standards of national and international accreditations. Manages the Hospital-wide incident reporting system and risk registry systems (Safety Reporting System).
13.Coordinates the investigation and conduction of Failure Mode and Effect Analysis (FMEA) and root cause analysis of actual and potential Serious/ Sentinel events in the Hospital.
14.Supports maintaining the Hospital as a High Reliability Organization. Participates in implementing quality management and quality improvement programs.
15.Participates in Performance Improvement (PI) teams, committees and taskforces as assigned.
16.Follows all Hospital policies and procedures.
17.Participates in self and other's education, training and development, as applicable.
18.Performs other related duties as assigned.Education
Master's or Bachelor's Degree in Nursing, Quality or other healthcare related discipline is required. Experience Required
Six (6) years of related experience with Master's, including two (2) years at a managerial position or eight (8) years with Bachelor's Degree is required. Other Requirements(Certificates)
.Certified Professional in Patient Safety (CPPS) or Certified Professional in Risk Management (CPHRM) is required.
.Certifications in Lean, Six Sigma and/ or other PI methodologies are preferred.
